Easy to fill out the form but its worth maybe $25. at best. It was not clear that the $88 I paid would do nothing but give me the opportunity to print the form that I filled out with what amounts to a postage stamp. I didn't need that help, I thought I was paying for a new passport. I informed them that I had made a mistake and wanted to do the online process and asked for a refund and they stopped replying to the email correspondence chain which is really sketchy.

Simple and clear instructions on template and on the printed forms. Mailing label included with tracking information. Easy process. Got my passport 2 weeks after mailing the packet in.
