My agency requires a W9 to set someone up as a vendor if the purchase is over $1000. After three weeks, I have not received any W9. Great product and the people are friendly. I will definitely be ordering from them again, I just have to go against our policy.

I have macular degeneration and had a stage three full thickness, macular hole that was repaired in one of my eyes. And no other lenses give my eyes the best vision and clarity in comfort than Oakley‘s prisms.
